• 应用程序 may be picked up in person from the guard's desk at SI's main entrance, or downloaded (links above)
• The application includes a teacher observation form that must be filled out by the student's current fifth grade teacher and submitted to FSA by the application deadline.
• A copy of the student's birth certificate

• 最近的标准化测试（四年级智能平衡评估可以接受）
• 一张学生近照和一张家庭近照
• 家庭经济援助信息表，包括在申请中
